Indications & Clinical Uses

Ossmiq is a broad‑spectrum triazole antifungal that targets fungal pathogens responsible for invasive disease.

  • Invasive aspergillosis: helps achieve clinical response and survival improvement in patients with hematologic malignancies.
  • Invasive candidiasis: contributes to eradication of Candida species in bloodstream infections.
  • Prophylaxis of fungal infection: reduces the incidence of fungal disease in high‑risk patients undergoing chemotherapy or stem‑cell transplant.

How It Works

  • Posaconazole inhibits the fungal enzyme lanosterol 14‑α‑demethylase, a key step in the ergosterol biosynthesis pathway. Blocking this enzyme disrupts cell‑membrane formation, leading to increased membrane permeability and fungal cell death. The drug exhibits activity against a wide range of yeasts and molds, including Aspergillus and Candida species.

Side Effects

Adverse reactions to posaconazole are reported across a spectrum of frequency.

Common Side Effects

  • Nausea and vomiting: mild to moderate gastrointestinal upset.
  • Diarrhea: loose stools that may be self‑limiting.
  • Headache: transient cranial discomfort.
  • Abdominal pain: cramp‑like discomfort in the stomach region.
  • Elevated liver enzymes: mild, reversible increases in ALT/AST.
  • Fatigue: generalised tiredness without specific cause.

Serious or Rare Side Effects

  • Hepatotoxicity: clinically significant liver injury requiring monitoring.
  • QT‑interval prolongation: cardiac rhythm changes that may predispose to arrhythmia.
  • Severe hypersensitivity: rash, angio‑edema or anaphylaxis.
  • Visual disturbances: blurred vision or photopsia.
  • Stevens‑Johnson syndrome: rare mucocutaneous blistering disorder.
  • Acute adrenal adrenal disorder (rare).

Precautions & Warnings

Posaconazole should be used with careful clinical oversight and patient monitoring.

  • Liver function monitoring: obtain baseline and periodic ALT/AST assessments.
  • Cardiac monitoring: ECG evaluation for patients at risk of QT prolongation.
  • Drug‑interaction review: assess concomitant medications that affect CYP3A4.
  • Pregnancy and lactation: avoid use unless benefits outweigh risks.
  • Renal impairment: adjust monitoring as posaconazole is primarily hepatically cleared.
  • Store at ambient temperature: keep in original packaging, protect from moisture and direct sunlight.

Avoid Interactions

Posaconazole has a high potential for pharmacokinetic interactions because it is a strong CYP3A4 inhibitor.

Major Interactions (Avoid)
