Ostaven (Alendronate Sodium)

Ostaven is a bisphosphonate that acts on bone metabolism disorders.

• Osteoporosis: Reduces the risk of vertebral and hip fractures in post‑menopausal women and men.
• Paget’s disease of bone: Helps normalize bone remodeling.
• Glucocorticoid‑induced osteoporosis: Lowers fracture risk in patients on long‑term steroid therapy.

• Alendronate sodium binds to hydroxy‑apatite crystals in bone, where it is internalized by osteoclasts during resorption. The drug inhibits the enzyme farnesyl pyrophosphate synthase in the mevalonate pathway, reducing osteoclast activity and promoting apoptosis, which leads to decreased bone turnover and a net gain in bone mineral density.

Adverse reactions to alendronate may range from mild to serious.

Common Side Effects

• Upper gastrointestinal irritation: heartburn, dyspepsia, or nausea.
• Musculoskeletal pain: joint or muscle aches.
• Headache.
• Transient flu‑like symptoms.

Serious or Rare Side Effects

• Osteonecrosis of the jaw (ONJ).
• Atypical femoral fractures.
• Severe esophageal ulceration or erosion.
• Acute hypersensitivity reactions, including anaphylaxis.

When prescribing Ostaven, clinicians should consider several safety aspects.

• Assess renal function: dose adjustment may be needed in impaired kidneys.
• Ensure proper administration: take with a full glass of water on an empty stomach, remain upright for at least 30 minutes.
• Review concomitant medications: avoid concurrent use with NSAIDs or oral steroids.
• Monitor calcium and vitamin D status: supplement if deficient.
• Store at below 25°C: keep in the original packaging, protect from moisture.
