Osteoteri (Teriparatide)

Osteoteri is a parathyroid hormone analog that acts on bone remodeling pathways to improve skeletal strength.

• Postmenopausal osteoporosis: increases bone mineral density and reduces fracture risk.
• Glucocorticoid‑induced osteoporosis: promotes bone formation to counteract steroid‑related bone loss.
• Severe osteoporosis at high risk of fracture: helps achieve greater bone mass when other therapies are insufficient.

• Teriparatide binds to the PTH1 receptor on osteoblasts, activating intracellular cyclic AMP and protein kinase pathways that stimulate osteoblast proliferation, differentiation, and activity. This leads to increased bone matrix synthesis and mineralisation, resulting in net bone formation and improved bone strength.

Teriparatide may cause a range of adverse reactions, most of which are mild to moderate in intensity.

Common Side Effects

• Nausea: occasional upset stomach or feeling of queasiness.
• Headache: mild to moderate head pain.
• Dizziness: transient light‑headedness.
• Injection‑site reactions: redness, swelling or bruising at the injection site.
• Hypercalcaemia: elevated blood calcium levels, usually mild.

Serious or Rare Side Effects

• Osteosarcoma: a very rare bone cancer observed in animal studies.
• Severe hypercalcaemia: markedly high calcium that may require medical intervention.
• Hypersensitivity reactions: rash, itching, or anaphylaxis in rare cases.
• Cardiovascular events: rare reports of hypertension or arrhythmia.
• Persistent leg pain: unexplained, ongoing discomfort in the lower limbs.

Before starting Osteoteri, clinicians should assess patient history, laboratory values and risk factors.

• Contraindication: patients with a history of osteosarcoma or skeletal metastases.
• Monitoring: regular serum calcium and vitamin D levels during therapy.
• Duration limit: treatment should not exceed 24 months to minimise long‑term risk.
• Pregnancy and lactation: avoid use unless the potential benefit outweighs risk.
• Renal function: adjust evaluation for patients with severe kidney impairment.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Teriparatide may interact with other medicines that affect calcium metabolism or bone turnover.

Major Interactions (Avoid)

• Bisphosphonates: concurrent use can blunt the anabolic effect of teriparatide.
• Calcium supplements high‑dose: may reduce drug absorption when taken simultaneously.

Moderate Interactions (Monitor Closely)

• Vitamin D analogues (calcitriol): may increase risk of hypercalcaemia.
• Loop diuretics: can potentiate calcium elevation.
• Corticosteroids: may require closer monitoring of bone markers.
• Anticonvulsants (phenytoin, phenobarbital): may alter vitamin D metabolism affecting calcium balance.
