Otospon (Ciprofloxacin, Fluocinolone)

Otospon combines ciprofloxacin and fluocinolone to address bacterial and inflammatory conditions of the ear. It is indicated for:

• Acute otitis externa: reduces bacterial load and inflammation, promoting faster symptom relief.
• Chronic otitis media with effusion: helps control infection and inflammation in the middle ear.
• Post‑surgical ear care: prevents bacterial colonisation and reduces postoperative inflammation.
• External auditory canal dermatitis: alleviates inflammatory skin changes while treating secondary bacterial infection.
• Eustachian tube dysfunction related otitis: assists in managing infection‑driven inflammation.
• Ear canal fungal fungi (only if bacterial superinfection present): provides antibacterial coverage alongside anti‑inflammatory action.

• Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication and leading to bacterial cell death. Fluocinolone, a synthetic corticosteroid, binds to intracellular glucocorticoid receptors, modulating gene transcription to suppress pro‑inflammatory cytokines, reduce vascular permeability, and diminish oedema in ear tissues.

Otospon may cause local and systemic adverse reactions. Patients should be monitored for any signs of intolerance.

Common Side Effects

• Ear itching or mild irritation.
• Burning sensation in the ear canal.
• Temporary hearing changes.
• Mild headache.
• Localised redness or swelling.
• Dryness of the ear canal.

Serious or Rare Side Effects

• Severe ototoxicity or hearing loss.
• Allergic reaction such as rash, urticaria, or angio‑edema.
• Persistent otorrhea with foul odor.
• Secondary fungal infection of the ear.
• Systemic hypersensitivity reactions.
• Vision changes or ocular irritation if product contacts eye.

Before using Otospon, consider the following safety measures:

• Contraindication: Do not use if there is a known hypersensitivity to ciprofloxacin, fluocinolone or any excipients.
• Pregnancy: Use only if clearly indicated.
• Breastfeeding: Caution advised; assess risk‑benefit.
• Auditory function: Monitor hearing if treatment exceeds 7‑7 days.
• Ear surgery: Avoid use in perforated tympanic membrane unless directed by a specialist.
• Store at room temperature: keep in original packaging, protect from moisture and direct sunlight.

Otospon may interact with other otic medications and systemic agents. Review :

Major Interactions (Avoid)

• Co‑administration with aminoglycoside otic drops : increased risk of ototoxicity.
• Concurrent systemic fluoroquinolones : potential additive tendon‑related effects.
• Use with topical non‑steroidal anti‑inflammatory drugs (NSAIDs) : reduced anti‑inflammatory efficacy.

Moderate Interaction : 

• Systemic corticost steroids : enhanced systemic steroid exposure.
• Anticoagulants : possible increase in bleeding risk.
• Anticholinergic : potential dry ear effects .
