Indications & Clinical Uses

Formoterol fumarate dihydrate is a bronchodilator that acts on the airway smooth‑muscle pathway to improve airflow.

  • Asthma: improves long‑term control and reduces symptoms in adults with persistent asthma.
  • Chronic obstructive pulmonary disease (COPD): provides maintenance bronchodilation and reduces exacerbation risk in adults with COPD.
  • Exercise‑induced bronchoconstriction: helps prevent airway narrowing during physical activity in susceptible individuals.

How It Works

  • Formoterol binds selectively to β2‑adrenergic receptors on airway smooth‑muscle cells, activating adenylate cyclase and increasing intracellular cyclic AMP. Elevated cAMP leads to relaxation of bronchial smooth muscle, producing sustained bronchodilation that lasts up to 12 hours after inhalation.

Side Effects

Formoterol may cause a range of adverse reactions, most of which are dose‑related and reversible.

Common Side Effects

  • Throat irritation: mild discomfort or dryness in the throat.
  • Cough: transient coughing after inhalation.
  • Headache: mild to moderate headache.
  • Tremor: fine muscle tremor, often of the hands.
  • Palpitations: sensation of rapid or irregular heartbeat.
  • Nasal congestion: mild blockage of the nasal passages.

Serious or Rare Side Effects

  • Paradoxical bronchospasm: sudden worsening of breathing difficulty.
  • Cardiac arrhythmia: irregular heart rhythm that may require medical attention.
  • Hypokalemia: low blood potassium levels detectable on laboratory testing.
  • Severe allergic reaction: rash, itching, swelling, or difficulty breathing.
  • Hyperglycaemia: elevated blood glucose, particularly in diabetic patients.
  • Pulmonary hypertension worsening: potential aggravation of existing pulmonary hypertension.
