Indications & Clinical Uses

Risedronate is indicated for disorders that affect bone remodeling and density.

  • Osteoporosis: improves bone mineral density and reduces risk of vertebral and hip fractures.
  • Paget's disease of bone: normalises abnormal bone turnover.
  • Glucocorticoid‑induced osteoporosis: helps maintain bone strength in patients receiving long‑term corticosteroid therapy.

How It Works

  • Risedronate binds selectively to hydroxy‑apatite crystals at sites of active bone remodeling. When osteoclasts attach to these crystals, the drug is internalised and inhibits farnesyl pyrophosphate synthase, a key enzyme in the mevalonate pathway, leading to reduced osteoclast activity, apoptosis and ultimately decreased bone resorption.

Side Effects

Adverse reactions to risedronate are reported across a range of severity.

Common Side Effects

  • Upper gastrointestinal discomfort: heartburn, dyspepsia or abdominal pain.
  • Musculoskeletal pain: transient bone, joint or muscle aches.
  • Headache: mild to moderate intensity.
  • Nausea: occasional feeling of queasiness.
  • Skin reactions: mild rash or erythema.

Serious or Rare Side Effects

  • Osteonecrosis of the jaw: exposed bone in the oral cavity, often after dental procedures.
  • Atypical femoral fracture: low‑energy fracture of the subtrochanteric region.
  • Severe esophagitis: ulceration or inflammation of the esophagus.
  • Acute phase reaction: fever, chills or flu‑like symptoms after the first dose.

Precautions & Warnings

When prescribing risedronate, clinicians should consider several safety aspects.

  • Renal function: assess creatinine clearance before initiating therapy.
  • Gastro‑intestinal risk: avoid in patients with active peptic ulcer disease or severe esophagitis.
  • Calcium/Vitamin D status: ensure adequate supplementation to support bone health.
  • Concomitant medications: review for drugs that may irritate the esophagus.
  • Pregnancy and lactation: contraindicated unless clearly needed; discuss risks with the prescriber.
  • Store at below 25°C: keep in the original container, protect from moisture and direct sunlight.

Avoid Interactions

Risedronate may interact with several medicines, altering its absorption or increasing toxicity.

Major Interactions (Avoid)

  • Calcium supplements: concurrent intake reduces risedronate absorption; separate by at least 2 hours.
  • Antacids containing aluminium or magnesium: may bind the drug and lower bioavailability.
  • Oral corticosteroids: increase risk of gastrointestinal irritation.

Moderate Interactions (Monitor Closely)

  • Non‑steroidal anti‑inflammatory drugs (NSAIDs): may heighten upper‑GI adverse events.
  • Proton pump inhibitors (PPIs): potential additive effect on esophageal irritation.
  • Diuretics (loop or thiazide): monitor electrolytes, especially calcium levels.
