Pamidronate Disodium is indicated for conditions that involve excessive bone turnover or calcium dysregulation.

  • Malignancy‑related hypercalcaemia: helps lower serum calcium to safer levels.
  • Osteolytic bone metastases: reduces risk of pathological fractures and bone pain.
  • Paget’s disease of bone: normalises bone remodeling and alleviates pain.
  • Prevention of bone loss after stem‑cell transplantation: supports maintenance of skeletal integrity.
  • Hypercalcaemia of immobilisation: assists in controlling calcium rise during prolonged bed rest.

How It Works

  • Pamidronate binds with high affinity to hydroxy‑apatite crystals on bone surfaces, especially at sites of active resorption. When osteoclasts attach to the drug‑coated bone, intracellular uptake occurs, leading to inhibition of farnesyl‑pyrophosphate synthase within the mevalonate pathway. This blocks prenylation of small GTP‑binding proteins, impairing osteoclast maturation, attachment and resorption activity, thereby reducing bone turnover.
