Indications & Clinical Uses

Pamidronate Disodium is indicated for several bone‑related conditions that involve excessive bone turnover.

  • Hypercalcaemia of malignancy: helps reduce elevated calcium levels caused by cancer.
  • Osteolytic bone metastases: slows the progression of bone lesions and reduces skeletal‑related events.
  • Paget’s disease of bone: normalises bone remodeling and alleviates pain.
  • Prevention of hypocalcaemia after parathyroidectomy: stabilises calcium levels post‑surgery.
  • Hypercalcaemia associated with hyperparathyroidism: assists in lowering serum calcium.
  • Bone pain associated with multiple myeloma: provides analgesic benefit through bone turnover reduction.

How It Works

  • Pamidronate binds to hydroxy‑apatite crystals on bone surfaces and is internalised by osteoclasts during resorption. Inside the cell it inhibits farnesyl pyrophosphate synthase, a key enzyme in the mevalonate pathway, which disrupts prenylation of small GTP‑binding proteins. This impairs osteoclast attachment, reduces bone resorption and ultimately leads to decreased release of calcium from bone.
