Indications & Clinical Uses

Ceftriaxone is a broad‑spectrum antibiotic that acts on Gram‑negative and Gram‑positive bacteria in several clinical contexts.

  • Community‑acquired pneumonia: helps eradicate the causative bacterial pathogens and improve respiratory symptoms.
  • Uncomplicated urinary tract infection: eliminates the infecting organism and resolves urinary symptoms.
  • Gonorrhoea: provides a single‑dose cure by targeting Neisseria gonorrhoeae.
  • Bacterial meningitis: penetrates the cerebrospinal fluid to treat the infection and reduce inflammation.
  • Skin and soft‑tissue infections: clears the bacterial cause and promotes wound healing.

How It Works

  • Ceftriaxone inhibits bacterial cell‑wall synthesis by binding to penicillin‑binding proteins, disrupting peptidoglycan cross‑linking. This leads to osmotic instability and cell lysis, which kills susceptible Gram‑positive and Gram‑negative organisms. The drug’s high protein binding and long half‑life allow sustained concentrations in serum and tissues.

Side Effects

Ceftriaxone may cause a range of adverse reactions, most of which are mild and reversible.

Common Side Effects

  • Diarrhea: mild to moderate watery stools.
  • Nausea: occasional upset stomach.
  • Injection site pain: local discomfort after IV administration.
  • Rash: transient skin erythema.
  • Elevated liver enzymes: mild, transient increase in hepatic transaminases.

Serious or Rare Side Effects

  • Anaphylaxis: severe, potentially life‑threatening allergic reaction.
  • Clostridioides difficile colitis: severe diarrhea due to intestinal flora disruption.
  • Hemolytic anemia: immune‑mediated destruction of red blood cells.
  • Stevens‑Johnson syndrome: extensive skin detachment and mucosal involvement.
  • Renal impairment: worsening kidney function in susceptible patients.

Precautions & Warnings

Ceftriaxone should be used with careful clinical assessment and monitoring.

  • Renal impairment: adjust or monitor dosing in patients with reduced kidney function.
  • Hepatic dysfunction: use cautiously in patients with liver disease.
  • Biliary sludge/gallstones: monitor patients with prolonged therapy.
  • Neonates and infants <28 days: avoid due to risk of bilirubin displacement.
  • Hypersensitivity: discontinue if signs of allergic reaction appear.
  • Store at below 25°C: keep in the original packaging, protect from moisture and light.

Avoid Interactions

Ceftriaxone can interact with several medicines, altering efficacy or safety.
