Pankreal (Pancreatin)

Pankreal is a pancreatic enzyme preparation that acts on the digestive tract to improve nutrient breakdown.

• Cystic fibrosis: enhances digestion of fats, proteins and carbohydrates, reducing steatorrhea.
• Chronic pancreatitis: supplements deficient pancreatic enzymes, improving nutrient absorption.
• Post‑pancreatectomy: provides exogenous enzymes to support digestion after surgical removal of pancreatic tissue.
• Pancreatic exocrine insufficiency (PEI): restores enzymatic activity for better digestion and nutritional status.

• Pancreatin contains a mixture of lipase, amylase and protease enzymes that mimic the natural secretions of the pancreas. After oral administration, the enzymes are released in the duodenum where they hydrolyze dietary fats, carbohydrates and proteins into absorbable molecules, compensating for the patient’s deficient endogenous pancreatic output.

Adverse reactions to pancreatic enzyme products are generally mild and related to the gastrointestinal tract.

Common Side Effects

• Abdominal discomfort: mild cramping or bloating.
• Flatulence: increased intestinal gas.
• Nausea: occasional feeling of queasiness.
• Diarrhoea: loose stools.
• Constipation: reduced bowel movements.
• Skin rash: mild erythema at the application site.

Serious or Rare Side Effects

• Fibrosing colonopathy: rare narrowing of the colon associated with high‑dose therapy.
• Severe allergic reaction: anaphylaxis, angioedema or urticaria.
• Pancreatitis: inflammation of the pancreas.
• Persistent vomiting: ongoing emesis despite dose adjustment.
• Intestinal obstruction: blockage due to bezoar formation.
• Hyperuricaemia: elevated uric acid levels in susceptible individuals.

When using Pankreal, consider the following safety measures:

• Dose adjustment: tailor the dose to the individual’s dietary fat content and clinical response.
• Monitoring: regularly assess nutritional status, weight and stool fat content.
• Allergic history: avoid use in patients with known hypersensitivity to porcine‑derived products.
• Concomitant acid‑suppressive therapy: may reduce enzyme activation; dose may need modification.
• Pregnancy and lactation: limited data; use only if clearly needed.
