Indications & Clinical Uses

Pantoprazole is a proton‑pump inhibitor that acts on the gastric acid secretion pathway. It is indicated for:

  • Gastro‑oesophageal reflux disease (GERD): alleviates heartburn and promotes healing of the oesophageal lining.
  • Erosive oesophagitis: supports the healing of erosive lesions in the oesophagus.
  • Zollinger‑Ellison syndrome: controls excessive gastric acid production associated with this rare endocrine disorder.

How It Works

  • Pantoprazole selectively inhibits the H+/K+‑ATPase enzyme (the gastric proton pump) located on the parietal cell surface of the stomach lining. By binding covalently to the pump’s cysteine residues, it blocks the final step of acid secretion, leading to a prolonged reduction in gastric acidity and an environment that facilitates mucosal healing.

Side Effects

Pantoprazole may cause a range of adverse reactions.

Common Side Effects

  • Headache: mild to moderate intensity.
  • Diarrhoea: typically self‑limiting.
  • Nausea: occasional and usually transient.
  • Abdominal pain: mild discomfort.
  • Flatulence: increased intestinal gas.
  • Constipation: occasional bowel habit change.

Serious or Rare Side Effects

  • Clostridioides difficile infection: severe colitis presenting with watery diarrhoea.
  • Acute interstitial nephritis: kidney inflammation with reduced urine output.
  • Severe allergic reaction: rash, itching, swelling, or anaphylaxis.
  • Low magnesium levels (hypomagnesemia): muscle cramps or irregular heartbeat.
  • Fracture risk: possible reduction in bone mineral density with long‑term use.

Precautions & Warnings

When prescribing Pantoprazole, consider the following precautions:

  • Assess patient history for renal or hepatic impairment: adjust use accordingly.
  • Evaluate the need for long‑term therapy in osteoporosis risk patients.
  • Monitor electrolyte levels especially magnesium during prolonged therapy.
  • Review concomitant medications that may affect gastric pH and absorption of other drugs.
  • Store at ambient temperature: keep in the original packaging, protect from moisture and direct sunlight.
