Form 12A & 12B Online – Import Medicines for Personal Use (India)
Apply online for CDSCO permission to import medicines for personal use via SUGAM Portal. Get Form 12B (Import Permit) after approval.
Prerequisites (Before Applying Online)
Doctor's Prescription
- From Registered Medical Practitioner (RMP)
- Include: Drug name, strength, dosage, duration
- MCI/State Council Registration Number
- State why import is needed
Other: Valid ID (Aadhaar/Passport), Proforma Invoice (optional)

Step 1: Register/Login on CDSCO SUGAM Portal
- Go to: https://cdscoonline.gov.in
- Click “Register” → Select “Individual”
- Enter Aadhaar/Passport, email, mobile → Verify OTP
- Login with credentials
Step 2: Fill Form 12A Online
- Click “Import of Drugs for Personal Use” → “Form 12A”
- Fill:
- Applicant details (Name, Address, Contact)
- Drug name, strength, quantity, manufacturer
- Port of entry (e.g., Delhi IGI, Mumbai, Chennai)
- Purpose: Personal Use Only
- Upload Documents (PDF only, ~2MB each):
- Prescription (with RMP registration)
- ID proof (Aadhaar/Passport)
- Proforma Invoice (if available)
- Submit → Save your Application Number
Step 3: Get Form 12B (Permit)
- Typical processing: 1–3 working days (often faster)
- Once approved: Form 12B downloadable from your SUGAM account
- Download PDF → Print & carry

Step 4: Clear Customs
At airport/seaport customs:
- Submit Form 12B (printout)
- Provide prescription, invoice, ID
Present complete paperwork to avoid delays. Check most recent CDSCO instructions at the time of import.
